USA Healthcare Management is currently hiring LPN's, RN's, Case Workers, and Social Workers to fill a Care Management role for a large group of high risk members. This position is a very independent and flexible position, allowing you to create your own schedule and work with very little supervision once properly trained. This position is a non clinical care management position, that allows the CM to go into the house of the member and devise care plans to help implement better and healthier living. The CM will be responsible for monitoring progress and documenting each occurrence with member. The requirements for this position are listed below; you will need to meet all of the requirements in order to qualify for this position.
CANDIDATE REQUIREMENTS:
o ALL CANDIDATES MUST HAVE AT LEAST 1 YEAR OF EXPERIENCE OR MORE.
o Candidate must have High School Diploma or GED Certification.
o Candidate must have graduated from accredited Program (proof will be required)
o Active and Clean License and Background check
o Basic MS Office (Word, Excel, Outlook)
o Candidate MUST HAVE a computer with internet access as well as access to a printer.
o Candidate MUST HAVE reliable transportation.
o Candidate MUST HAVE knowledge of Managed Care as well as Medicaid/ Medicare and how they work.
o Candidate MUST HAVE Telephonic/ In Home customer service skills.
o Experience in Care Management, Skilled Nursing Facility, Geriatrics, Home Health Care, &/or Hospice is a plus.

o All candidates must provide a copy of professional license or degree as well as pass a background check.
o All candidates must sign an employment contract, code of conduct, pay rate agreement, as well as other work affiliated documents in order qualify for this position.
o Once preliminary screening process is completed, documentation training will be provided online to all Care Management professionals that have been hired.
o Additional training will be provided to all CM's as needed, to ensure all job requirements and procedures are understood.

oTo coordinate services for members in person and assist with community resources as well as treatment needs.
oFunctioning as an advocate for members and decreasing unnecessary hospitalizations.
oTravel in assigned territory as needed to complete patient assessments thoroughly.
oIdentifying early risk factors and conducting ongoing assessments and documenting in an electronic file.
oCollaborating with the member's PCP to deliver and coordinate necessary services.
oBuilding relationships with members and their families; assisting them with proper health choices.
oEnsuring cases are documented in a timely manner.
