Major Function
Social Service case management and assessment of needs for indigent county residents for assistance through field, office or home interviews and/or investigations for medical and financial assistance available through the Social Services Section.
Illustrative Duties
Employees in this class perform duties of more than average difficulty requiring the exercise of reasonable initiative and independent judgement under general supervision.
The following duties are illustrative and exhaustive. Depending on assigned area of responsibility, incumbents in the position may perform some or all of the activities described below.
Receives referrals, may make home visits and interviews clients and or family members for assessment of need for multiple program services and community resources. Investigates applicant's county eligibility status for medical and financial assistance; including eligibility and maintenance of Medical Needy Share of Cost program. Evaluates financial, social, environmental, medical, family status and related information making proper disposition or referral of case on basis of eligibility. Prepares and follows case plans as requested with the goal to return clients to a self-sustaining status. Counsels applications for referral to proper sources, developing community resources, and regarding county eligibility determinations of rules, regulations and laws relating to the Polk Healthcare Plan, Medically needy share of Cost Program and HIPPAA, Health Insurance Portability and Accountability Act.
Knowledge, Ability and Skills
o Working knowledge of Social Work theories, techniques and methods of case management procedures.
o Working knowledge of Federal, State, County, and community social service programs for which most clients may be eligible including eligibility requirements, application procedures, and other relevant program policies.
o Ability to become familiar with other social services agencies and resources.
o Ability to correctly apply applicable laws, regulations, and policies to individual cases.
o Skill in interviewing clients to identify deficiencies in personal, legal, social, economic, and health needs.
o Skill in crisis intervention techniques
o Skill in dealing with social service clients under stressful conditions.
o Ability to use good judgement and make independent decisions.
o Skillful writing, reporting, and record keeping abilities.

o Ability to research, investigate, experiment, detect, inspect, verify, recognize and recommend solutions.

o Ability to use a computer terminal to enter and extract information to accomplish research and reports.
o Knowledge of Microsoft Office 2010. Ability to quickly learn CareScope, Relay Health MCO or any other software systems utilized by the Polk Healthcare Plan.
Minimum Qualifications
Graduate of an accredited 4 (4) year college or university with major course work in Social Work, Sociology, Phycology or a related field and a minimum of one (1) year social service interviewing, counseling or related case management experience.
Must possess a valid driver's license and be able to secure a valid Florida driver's license at the time of employment.
A comparable amount of training and experience may be substituted for the minimum qualifications.

Don't Be Fooled

The fraudster will send a check to the victim who has accepted a job. The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
