At TrueCore Behavioral Solutions, our mission is to empower individuals, families, and communities to discover their true potential.
In this position you will provide transition services from the beginning to end of the program including assessment, coordinating with community networks, and providing a seamless connection and transition for the youth.
This includes providing compliance monitoring for 30 days following a release from the program to ensure the youth is meeting all criteria of his transition plan.
This will include collaboration with the youth, family, JPO and community support services.
This position requires compassion, empathy, patience, and a true passion for working with youth.
The goal is to help at-risk youth get out of their programs and become successful young adults with your help.
We use evidence-based practices and believe in quality treatment and improvement for our clients.
