The Social Service Specialist II is a member of a collaborative practice model that includes patients, nurses, hospital care coordinators, physicians, and other practitioners, caregivers, and the community.
Provides therapeutic interventions through assessment, Coordination, referral and interdisciplinary planning Knowledgeable about treatment, social and economic implications for individuals and families as it pertains to their age, stage of development and discharge needs.
Possess a clear understanding of discharge and transition planning and linkage with community resources.

Qualifications:
Education:
Master's Social Work Master's Sociology Experience 6 months Internship 1 year Social Work in an acute care or hospital setting.

Estimated Salary: $20 to $28 per hour based on qualifications.
